Subject: Another problem(s) with my Powerstroke. Any takers on what's wrong?

Truck:     2004 F-250, 4x4, Crew cab, SWB with 6.0l PSD and 115,000 miles

Problems, which my dealer swears that they can never get to happen when it's in the shop:

  1. The electric auxiliary cooling fan runs constantly and engine and tranny temps are normal, no matter the driving style (in-town, high-way or even when towing or not).
  2. Tranny will not shift until engine RPM gets to around 3,000 - 3,500 RPM, no matter if hot or cold
  3. Intermittent to no heat (a new problem)
Any idea what's wrong?

History of problems:

  1. Failed: throttle body (< 50,000 miles, replaced - warranty)
  2. Failed: engine oil cooler (< 50,000 miles, replaced - warranty)
  3. Failed: EGR oil cooler (101,000 miles, replaced - $2,000)
  4. EGR system rebuild (101,000 miles, fix-as-fail factory program)
  5. Failed: two injectors (> 50,000, replaced - warranty)
