We use OneNote. Allows you to create the check box in a list format and either print it for hard copy or use it digitally. We copy the sheet each year for the next. We went to including the hours/acres for equipment in the sheet - one spot for beginning year information, and then another spot for end of the season/year.
You can add excel spreadsheets, images, equations, checklists, word docs, etc...
