Hmmmmm..... possibly the current surge drawn by the phone when ringing / activating blew a component and made a short in the power supply? A switching power supply for charging, whose transformer is not completely isolated from the AC line? Perhaps the current surge overheated the battery, and exploded?

I thought the chances of getting a fatal shock are about the same as getting one from any other AC operated device, such as a toaster, TV, electric shaver.... etc.
