A couple of suggestions that I would have are:
1. Don't let it over-rise the first time. Double in size is the rule.
2. After you punch it down and reshape it, make sure you have a warm enough place for it to rise the second time. I cover it and try to put it in a sunny window. Or else I warm up the oven, then turn it off and put the bread in there for the second rising time. If your room is too cool it won't rise very well.
3. Make sure your yeast is good and not expired, or close to expired.
My first reaction would be that you let it over-rise in size and then had too cool of a space for the second rise time.
