Just build a steel frame and anchor it to the concrete. Then use a standard size door.

Building hollow metal frames is exactly what commercial door hardware companies do!!!!! If you think you can scratch build a frame, get the hinges in the right place, drill and tap hinge screw holes, get the strike plate in the right location, drill and tap holes for the strike plate, you work for free, the material is free, and you can make it remotely good looking, then I guess go for it. There is no way one can make a home made frame cheaper or better looking than the people that do it for a living. This is going in somebody's home, not a farm building. As I have stated twice above, the OP can put a "standard" 3-6 x 6-8 store bought HM frame with a 4" head in his existing opening.
