They are recommending 250 lbs/A/year P₂O₅ which in theory will improve the soil test by >< 10 ppm/year P. They seem to think your soil should be in the 40 to 60 ppm P range.

The potash should be 300 #/A/year K₂O. In theory 300 #K₂O should increase the soil test by 60 ppm K or bring it up to standard. Most soils do well with a soil test in the 150 to 200 ppm K range.

In theory you should be able to apply enough to bring the soil test values up to speed plus adding enough for the next crop.

Lime is slow to add calcium or the effect pH. You might use a split application. One with fines and the other with more chunks of lime. My friends in East TX like to apply fines for their lime. I suspect you will be applying lime again after the first 4 years.
